Florida Supervision Log - SW, MFT, MHC

The Florida's 491 Board requires qualified supervisors and supervisees use their Supervision Log [Form DH-MQA 5078, (5/2023)]. This Supervision Log is used for documenting supervision hours; both the registered intern (supervisee) and supervisor should have access to identical copies.
